Chronic diseases such as heart failure, cancer, diabetes, chronic arthritis and kidney failure affect one in 10 adults, leading to major limitations in daily living. Chronic disease accounts for 70% of deaths in North America, consumes a disproportionate amount of health care resources and results in untold amounts of human suffering. Kidney disease is a common form of chronic disease.
